What to Expect When Wisdom Teeth Come Through

Wisdom teeth are the last to arrive โ€” and the most likely to cause problems

Wisdom teeth (third molars) are the final set of teeth to develop, usually appearing between ages 17-25 โ€” though some people get them earlier, later, or not at all. About 25% of people never develop all four wisdom teeth.

Normal eruption symptoms

  • Pressure or aching at the back of the jaw
  • Slight swelling of the gum behind the last molar
  • Tenderness when chewing
  • A sensation of the gum being stretched

These symptoms are usually mild and intermittent as the tooth gradually pushes through. Over-the-counter painkillers and warm salt water rinses help.

When wisdom teeth cause problems

  • Impaction: The tooth is stuck against the tooth in front or growing at an angle. This is the most common problem โ€” it affects about 70% of people with wisdom teeth.
  • Pericoronitis: The gum flap over a partially erupted tooth traps food and bacteria, causing repeated infection. Symptoms: severe pain, swelling, difficulty opening the mouth, bad taste.
  • Crowding: Debated โ€” some orthodontists believe wisdom teeth contribute to crowding, others disagree.
  • Decay: Their position at the back of the mouth makes them hard to clean.

Do all wisdom teeth need removing?

No. If your wisdom teeth come through straight, have enough room, and you can keep them clean โ€” they can stay. Your dentist monitors them at routine check-ups and will recommend removal only if they are causing or likely to cause problems.
